Operating under the 2026 NDIS Support Needs framework, we specialise in moving participants toward Independent Living through a seamless, outcome-driven pathway of professional counselling and mentored support.

Our Unique Approach: The Acts Two Communities Partnership

We do not believe in isolated care. Through our Acts Two Communities Partnership, we work alongside local churches to create “Hubs” of belonging. This model allows us to identify individuals who are currently unserved by the system and provide them with the expert System Navigation required to access the NDIS. By embedding our participants within a church-based community, we meet the highest standards of Social and Economic Participation.

At HH&H, we utilise a High-Trust, Low-Friction operational model. By automating our financial and administrative workflows through our custom Google Workspace infrastructure, we ensure that our contractors spend less time on paperwork and more time on participant outcomes. Every shift note and progress report is designed to meet the 2026 Quality and Safeguards standards, providing the NDIA with the data integrity they require.
